Midland Care Co-ordination Forum
The Midland Care Co-ordination Forum is made up of cancer and palliative care clinical nurse specialists and patient navigators working across the hospital and community health sectors.
Its role is to
- contribute and support cancer care co-ordinators functioning and share information and good practices
- discuss issues and troubleshoot problems
- have input and work on relevant components of the Midland Cancer Network's work programme.
